Community Care's mission is to help elderly individuals and adults with physical and intellectual disabilities continue to live as independently as possible in the community. This full-time opportunity offers a weekday schedule and a $5,000 SIGN-ON BONUS. It is based in Racine County with telework currently being the norm.Case Manager/Care Manager Job Responsibilities:--- Manages an assigned caseload of program members in collaboration with an RN Care Manager--- Completes comprehensive member assessments, member centered care plans, & functional screens--- Provides ongoing care coordination and quality assurance monitoring of services--- Maintains required documentation in adherence to established standards.Case Manager/Care Manager Job Requirements:Education: - Bachelor's Degree in a Human Services Area.- A minimum of three (3) years experience working with seniors or adults with physical and/or intellectual disabilities my be substituted for a non-Human Services degree.Experience: One year of experience with older adults or persons with physical and or intellectual disabilities preferred.Certification: Eligibility for long-term care functional screener certification is requiredTravel: Regular local travel, requiring use of personal vehicle, valid driver's license, and proof of required insurance coverage.Care Manager Job Schedule:--- Monday to Friday--- Generally during business hours of operation (8AM-4:30PM)Community Care is a nonprofit organization with over 40 years of experience helping adults with long-term care needs live as independently as possible.
