We Are Hiring: Lakeshore at Siena is a beautiful 96-bed skilled nursing community and is a part of the Ascension family. When additional therapy is needed after surgery, illness or hospital stay, we provide rehabilitation and short-term care. We offer long-term care for residents who need a higher level of assistance in dealing with an ongoing illness, chronic medical condition or disability.
Also, within Lakeshore, Shorelight at Siena, offers Memory Care services. We subscribe to a philosophy of personalized care, which emphasizes each person's quality of life, dignity and comfort.

Schedule
3 days/week - 7.5-hour shifts
Every other weekend required
2nd Shift: 2:00pm to 10:30pm
What You Will Do: Provide direct nursing care to patients in the long term care setting in accordance with established policies, procedures and protocols of the healthcare organization.

• Implement and monitor patient care plans.
• Monitor, record and communicate patient condition as appropriate.
• Serve as a primary coordinator of all disciplines for well-coordinated patient care.
• Note and carry out physician and nursing orders.
• Assure resident safety by identifying safety hazards and emergency situations and initiate corrective action as appropriate.
• Assess and coordinate patient's discharge planning needs with members of the healthcare team. What You Will Need: Licensure / Certification / Registration:
  • BLS Provider obtained prior to hire date or job transfer date required.
  • Registered Nurse obtained prior to hire date or job transfer date required. Education:
