Job Description:
Qualifications include: Graduate of an accredited nurse
practitioner program and completion of Masters of Science Degree in
Nursing - OR Graduate of AMA accredited physician assistant
program. Current Wisconsin license as a Registered Nurse and
recognition as an Advanced Practice Nurse by the Wisconsin State
Boards of Nursing - OR Current license as a Physician Assistant by
the Wisconsin Boards of Medical Practice Current BLS and ACLS
Certifications Must possess active DEA Number.Schedule:2 days per
week in Baldwin, WI.Duties include:This role works in collaboration
with physicians and other medical staff to provide services to
patients and their families. They are engaged in independent
decision-making about health care needs and provides health care
services to individuals and their families. Performs a
comprehensive history, including chart review and information
gathered from the family and performs physical and psychological
health assessments. This role will also assist in the development
of a cancer survivorship program and serve as the primary clinical
provider in the survivorship clinic.Assess health status for
specific patient populations using advanced practice knowledge and
skillsPerforms clinical examinations and consultations with cancer
patients and cancer survivors seeking care. Include generation of
cancer treatment history for cancer survivors.Obtains relevant
health and medical historyPerforms a physical examination based
upon age and medical historyOrders and interprets necessary
procedures and diagnostic testsIdentifies physiological and
psychological conditionsIdentifies patients' symptoms and goals and
serves as an advocate with the health care providersSynthesizes
information from the history, physical and diagnostic tests to
develop, document and implement a Treatment Plan or Survivorship
PlanEstablishes a practice agreement with the collaborating
physicianCollaborates with the patient's primary physician in a
consultative roleCollaborates with all of the patient's physicians
and other health care team members in developing realistic
treatment/survivorship goalsDevelops a problem list based upon
physiological and psychological disordersDevelops and implements a
treatment/survivorship planCommunicates effectively with patients
and families for understanding of diagnosis, prognosis and
planDetermines the effectiveness of the treatment/survivorship plan
and documents patient care outcomesRevises and modifies the
treatment/survivorship plan to meet the patient's goalsKnowledge of
pharmacology principles. Prescribes pharmacological (including
controlled substances) and non-pharmacological (including
controlled substances) and non-pharmacological agentsDevelops and
Promotes Survivorship Program to patients, families, community, and
other health care professionalsAssists with writing, identifying,
and implementing program eligibility guidelines, evident based
protocols, standardized procedures and patient
questionnairesDevelops relationships with other specialties that
can assist with the management of cancer survivor problemsProvides
education to patients, families or caregivers as neededProvides
continuing education to community and other professional groups as
requestedCollaborates and coordinates with other health
professionalsMakes and accepts referrals to//from, consults with,
cooperates with and refers to all other types of healthcare
providers, including primary and specialty care physicians, nurse
practitioners and other allied personnelDiscriminates between
situation requiring independent management, collaborative
management with physician and further referralCollaborates with
physician when unusual incidents or changes in patient's condition
occurCoordinates care involving other healthcare providersFunctions
as mentor/resource and educator for other healthcare team and
