CONTRACT:
---- Dates of contract: 9/19/2022 to 12/18/2022 (14 weeks)
---- Shift: 8:00AM to 5:00PM, - Mon-Fri
---- Pay Rate: $52/hr.
---- Guarantee 40 hours

QUALIFICATIONS:

1+ years of experience or equivalent
Active License
NBCOT verification
Dementia Training
References
In-Service Acknowledgement
Covid vaccination & Covid Test
Influenza vaccination or declination
Physical
TB Screen/Chest X-Ray/QuantiFERON
Ability to pass a Background check

Physical stamina to lift patients and spend long hours standing and modeling exercises.
Interpersonal skills to build positive relationships with patients and motivate them.

RESPONSIBILITIES:

Ability to review and evaluate patient's medical history, conditions and needs
Assessing the patient's physical, communication and cognitive skills to develop a treatment plan.
Personal attention and progressive restorative therapy to achieve highest level of recovery and function in daily tasks.
Giving advice and providing support for family members and Caregiver
Teach therapeutic exercises, techniques and stretches to improve condition.
Installing the necessary equipment in the classroom for children with disabilities and teaching them how to use the equipment
Helping adult patients to take part in everyday activities based on their unique circumstances
Assisting patients' exercise for pain relief
Maintain patient records by keeping track of goals and progress while reviewing the patient's progress and adapting treatment plans accordingly.
Recommend adaptive equipment's and instruct patients on their uses.
Performing administrative tasks such as keeping records, writing reports and making telephone calls
Assist other facility staff members as needed to provide care and create a clean, orderly and effective environment for rehabilitation and physical therapy.
