and comfort.Responsibilities:Demonstrates excellent verbal and
written communication skills with patients/families and fellow
peersDemonstrates leadership skills in development and updating the
patient's plan of care.Demonstrates empathy in the delivery of
patient centered careCompletes, maintains, and submits accurate and
relevant timely clinical notes regarding patient's condition and
care given.Provides professional nursing care buy utilizing all
elements of the nursing process.Assess and evaluate patient
status.Completes nursing assessments utilizing critical analytical
skills with strong support from the IDG teamIndividualized
orientation based on previous experienceInitiate and revise patient
plan of care.Completes, maintains, and submits accurate and
relevant clinical notes regarding patient's condition and care
given.Participates in on-call duties as defined by the on-call

Qualifications:Licensed as a Registered Nurse in the state of
Wisconsin and be able to travel within a 25 mile radiusCPR
certifiedPrior Nursing experience in a TCU, Hospice, or Home Health
setting preferredValid Driver's License and reliable vehicle with
current insuranceExcellent at planning, organizing, and
communicatingDemonstrated ability to work autonomously #SJ
