Job Description:
At Crossbridge Hospice, we are dedicated to providing meaningful
end-of-life care to patients and indispensable support to their
families. As a Community Liaison, you will be pivotal in connecting
our hospice services with the communities we serve, ensuring
awareness and accessibility of our compassionate care options. Your
role will involve collaborating with local healthcare providers,
organizations, and community groups to foster partnerships that
enhance patient outreach and service delivery. You will be the face
of Crossbridge in the community, embodying our core values and
commitment to compassionate care. Develop and maintain
relationships with local organizations, healthcare providers, and
community groups to promote hospice services. Organize and
participate in community events to raise awareness about hospice
care options and Crossbridge services. Coordinate with healthcare
professionals and community leaders to facilitate referrals and
improve service accessibility. Gather feedback from the community
to continuously improve services and address community needs.
Conduct educational sessions and presentations about the benefits
and services provided by Crossbridge Hospice. Strong interpersonal
and communication skills Ability to network effectively Experience
with public speaking and presentations Understanding of healthcare
services and community resources Skills in organizing and
coordinating events Certification in Community Health, Public
Relations, or a related field is advantageous but not required.
